29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about January 23, 2020, the employer failed to ensure that an employee, who was using an electric powered nail gun, was wearing protective safety glasses.  An employee who was using the electric powered nail gun on a residential roof project, lated at 431 Amberley Dr., Uniontown OH, was exposed to flying scrap and nails.

There was no personal fall arrest system being worn or alternative fall protection system in place.

The employee was performing roofing activities on a new residential roof located at 431 Amberley Dr., Uniontown OH.  There was no personal fall arrest system being worn or alternative fall protection system in place.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about January 23, 2020, the employer failed to ensure that a portable ladder, which was being used to access a residential roof at about 15 feet high at the upper landing point, was extended at least 3 feet above the upper landing surface to provide stability for getting onto and off of the roof.  The two observed ladders were placed nearly flush with the upper landing point.
