29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about January 23, 2020, on the north exterior elevation of a structure under construction located at 650 NW Fork Road in Stuart, Florida, an employee was exposed to a struck-by eye injury when operating a pneumatic nail gun while not wearing eye protection equipment.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  On or about January 23, 2020, on the north exterior elevation of a structure under construction located at 650 NW Fork Road in Stuart, Florida, employees were exposed to fall hazard of approximately 15-feet 3-inches when installing siding to the exterior of a building from the basket of an aerial lift while not being protected from falling to the lower level.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:   On site of a structure under construction located at 650 NW Fork Road in Stuart, Florida, employees were exposed to a fall hazard when performing siding installation work from the basket of an aerial lift prior to being provided with training that would enable them to recognize the hazards associated with falling while using the lift.
