29 CFR 1926.453(b)(2)(v): A body belt or harness was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated.  On or about January 15, 2020, and times prior thereto, employees using an aerial lift without the use of a full body harness and lanyard attached to the basket were exposed to unprotected fall hazards of approximately 25 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about January 15, 2020, and times prior thereto, the employer did not provide employees exposed to fall hazards a training program and training each employee in the recognition and elimination of fall hazards.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  On or about January 15, 2020, and times prior thereto, the employer did not provide a certification record to verify training for employees exposed to fall hazards.
