29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a) Warehouse - Employees exposed to chemicals such as but not limited to: sulfuric acid and Clorox bleach, did not have the ability to access a functioning eye wash station due to the station not having any liquid and has not been inspected for two years, on or about January 23, 2020.     Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    a) Black Production - The employer did not ensure that the 2HP water injection pump was directly wired for continuous use, exposing employee to electrical shock hazards, on or about January 23, 2020.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  a)  94 Leistritz tower (White Room) - Two electrical cords were connected into a 4 way junction box to supply power to additional mixing components of the pump machine without strain relief, on or about January 23, 2020.  b)  Industrial Battery Charger - Male end of the industrial battery charger used to charge Toyota tow motors had a torn jacket exposing wires to employees gaining access to materials located on a shelf above, on or about January 23, 2020.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
