On or about January 15, 2020, employees were exposed to a fall of approximately 10 feet while performing masonry activities on a new school located at 449 Richmond Road, Richmond Heights, Ohio 44143. The employees were working from a Spider Satellite Mast Climber Scaffold and the employer did not ensure that each scaffold component could support four times the intended load.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. On or about January 15, 2020, employees were exposed to a fall of approximately 10 feet while performing masonry activities on a new school located at 449 Richmond Road, Richmond Heights, Ohio 44143. The employees were working from a Spider Satellite Mast Climber Scaffold and the employer did not ensure that each employee was trained to control and minimize the hazards associated with the scaffold.
