Drainage was not maintained where wet process(es) were in use: a) Washer area, 198 Gressly Road, Ruffs Dale, PA 15679: Observed on or about January 8, 2020, a machine used to wash cylinders was leaking water onto the floor. The water collected in a puddle between the machine and a garage door. Employees working in the area were exposed to slip hazards.

29 CFR 1910.305(g)(1)(iv)(B): Flexible cords were run through holes in walls, ceilings, or floors: a) Facility, 198 Gressly Road, Ruffs Dale, PA 15679: Observed on or about January 8, 2020, a flexible cable was run from a wall outlet (110 volt) located to the left of an electrical panel marked '9-5', through a hole in the wall, to an area outside of the building. The flexible cable was being used to provide power equipment in the grind shack area.
