29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: Back Room, South Exit Door. On or about January 24, 2020, the exit door had a lock affixed on the inside of the door with a key inserted into the lock. To exit through the door, employees had to turn the key, remove the lock and then push the panic bar to open the door. Employees were exposed to burns and smoke inhalation from obstructed or delayed egress in the event of a fire. There is no abatement documentation or certification required for this item.

Entire facility. On or about January 21 through 27, 2020, the water in the establishment had been turned off by the City of Morris for non payment of the establishment's water bill. Employees did not have access to potable water for drinking, washing hands, or use of bathroom facilities, etc. Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment. Back Room, South Wall. On or about January 24, 2020, the employer did not maintain at least 36 inches of clear space in front of electrical panels, including circuit breaker panels. Employees accessed one of the panels to turn lights on and off. Access and working space about the panels was partially restricted by storage of materials such as a ladder and tool chest. Employees were exposed to electrical hazards and delayed shutoff of equipment in an emergency. There is no abatement documentation or certification required for this item.
