A portable ladder stand used by employees to access the top of the ribbon mixer required employees to step over 2 from the top platform of the portable ladder stand to the top of the ribbon mixer.  Employees using the portable ladder stand to access the top of the mixer were exposed to a fall hazard of approximately 7 feet to the concrete floor below.
