Bacom Inc., At a worksite located at 1280 London Groveport Road, Lockborne, Ohio, 43137: On and/or preceding January 23, 2020, the employer did not ensure that employees engaged in roof decking activities with a fall hazard of 30 feet or more were protected from falls. An employee was shaking out and making the initial securement of roof decking to the top of the roof joists and girders when the employee fell 43 feet, 8 inches to the ground below.
