OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burn hazards from machinery/equipment failures:    A.)  Triangle Suspension Systems, Inc., 200 East Maloney Road, DuBois, PA  15801, on or about January 15, 2020 and times prior thereto:    An employee was operating a Heuser AFS2 (Serial No. 1733) Automatic Spring Eye Forming Machine to form the eye of a suspension system leaf spring.  During the gripper cylinder rotation to form the eye of the leaf spring around the mandrel, the bulkhead fitting became loose causing it to change position putting tension on the hydraulic hose and its associated rotary union.  The tension caused a metal fitting between the rotary union and the gripper cylinder to crack open, spraying a hydraulic oil mist over the hot piece of metal being formed.  The hot metal ignited the hydraulic oil mist resulting in a flash fire, which caused burns injuries and injuries associated with a fall at ground level to the employee.
