29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts, ingoing nip points and flying fragments:    (a) For employees leased to Ace Partnership LLC dba Crown Engineering Corporation, Production, Tool & Die Room:  The Clausing lathe, model 6903,  serial # 600270S, was not provided with a guard over the rotating chucks, on or about 1/15/2020.    (b) For employees leased to Ace Partnership LLC dba Crown Engineering Corporation, Production, Tool & Die Room:  The South Bend lathe, catalog no. CL187ZB , was not provided with a guard over the rotating chucks, on or about 1/15/2020.    (c) For employees leased to Ace Partnership LLC dba Crown Engineering Corporation, Tool & Die Room: The belt sander/grinder, BS 203, was not provided with a guard over the open side to protect the operator from ingoing nip points and rotating parts, on or about 1/15/2020.    (d) For employees leased to Ace Partnership LLC dba Crown Engineering Corporation, Production, Terminal Department: The Walker Turner drill press, T-8, model #1226-61, serial #0000005, was not provided with a guard to over the chuck to protect the operator form rotating parts and ingoing nip points, on or about 1/15/2020.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

(b) Tool & Die Room: The belt sander/grinder, BS 203, was not secured to prevent moving, on or about 12/16/2019.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which described how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) would be met:    a) For employees leased to Ace Partnership LLC dba Crown Engineering Corporation, Facility: The employer did not develop and implement a written hazard communication program for the chemicals used at the facility, such as, but not limited Valker Fast Drying Degreaser,  carcinogen, and Safety Silv 25 on or about 1/15/2020.     NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASE FILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION OR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
