29 CFR 1910.110(e)(4)(iii): Permanent and removable fuel containers were not securely mounted to prevent jarring loose, slipping, or rotating:  JOB SITE: 108 Business Highway 61, Bowling Green, MO 63334, on or about 1/28/2020, the employer failed to ensure that removable LP-Gas containers on a Yale Propane Fueled Lift, MODEL: GLP 050QXNVRE086, S/N: B875V01719F were securely mounted on powered industrial trucks to prevent displacement.  Powered industrial trucks were permitted in operation without the necessary cylinder indexing pin.

The flexible cord was used to power portable hand tools with a missing ground pin.  The workers were exposed to an electrical hazard.
