29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling: a) Job site, 4501 Ohio River Boulevard, Bellevue, PA: On or about January 15, 2020, during gutter cleaning and/or communication line repair work on top of the canopy, the employer did not ensure that appropriate fall protection systems/devices were in use in that the metal panels that were located around the perimeter only extended approximately 18 inches above the top of the canopy, exposing employees to fall hazards of up to at least approximately 13 feet 6 inches.
