29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about January 24, 2020, the employer did not establish an energy control program consisting of energy control procedures, training and periodic inspection. Employees were exposed to an unexpected start up of equipment and/or unexpected release or energies such as electricity and an arc flash. Employees were exposed to the hazards of an electrical shock and/or arc flash injuries.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:   On or about January 24, 2020, and times prior thereto, the breakers in the panel box located outside the building that would control the MCC breakers were not legibly marked to indicate the purpose. The breakers go to a MCC unit, which had the breaker for the Fulton furnace. Employees exposed to an electrical shock hazard and delay of isolating and de-energizing the electricity.

29 CFR 1910.335(b)(1): Safety signs, safety symbols, or accident prevention tags were not used where necessary to warn employees about electrical hazards which may endanger them as required by 29 CFR 1910.145.  On or about January 24, 2020, and times prior thereto, the employer failed to ensure the signs indicating the potential for an arc flash hazard could exist and the required personal protective equipment that should be worn. Employees were exposed to the hazards of an electrical shock and/or arc flash injuries.

29 CFR 1910.333(a): Safety-related work practices shall be employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work is performed near or on equipment or circuits which are or may be energized. The specific safety-related work practices shall be consistent with the nature and extent of the associated electrical hazards:  On or about January 24, 2020, the employer did not establish safety-related work practices procedures when working on or around live conductors. Employees were replacing a 480 volt  MCC breaker for Fulton furnace. Employees were exposed to the hazards of an electrical shock and/or arc flash injuries.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:   On or about January 24, 2020, the employer failed to ensure that employees permitted to work on electrical system were given the proper personal protective equipment, such as but not limited to, flame resistant clothing, and arc flash personal protective equipment. Employees removing 480 volt breakers. Employees were exposed to the hazards of an electrical shock and/or arc flash injuries.
