On or about January 28, 2020, and at times prior thereto, employees were exposed to struck-by hazards while operating a Hyster 155 Fortis with an Caldwell LIF-TRUC Boom, Model Number PB-60, attachment without the manufacturer's written approval. b. On or about January 28, 2020, and at times prior thereto, employees were exposed to struck-by hazards while operating a Linde Model H40D with a KOKE Man Basket, Model Number MBF-4848ND, attachment without the manufactures written approval.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however, the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a. On or about January 28, 2020, and at times prior thereto, employees were exposed to struck-by hazards while operating a Hyster 155 Fortis with a Caldwell LIF-TRUC Boom, Model Number PB-60, attachment that was not marked to identify the adjusted load capacity. b. On or about January 28, 2020, and at times prior thereto, employees were exposed to struck-by hazards while operating a Linde Model H40D with a Koke Man Basket, Model MBF-4848ND, attachment that was not marked to identify the adjusted load capacity.
