29 CFR 1910.28(b)(3)(i): (3) Holes. The employer must ensure: (i) Each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following: a) Penn Mechanical Group, Inc., at a worksite located at 66541 Country Club Road, St. Clairsville, Ohio 43950: On and/or preceding January 19,2020, the employer did not ensure that employees who were spotting the removal of a Blow Out Preventer were protected from falls. Three out of four covers were not in position over the oil rig cellar, resulting in an employee falling into the cellar with injuries requiring hospitalization.
