29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  At a jobsite located at 15912 East US 24 Highway, Independence, Missouri 64050, an employee was exposed to serious struck by, slip and fall, and engulfment hazards. The ladder which was being used for access and egress into the trench was an Werner A-frame ladder. The Werner A-frame ladder was folded closed and was leaning on the face of the trench.
