29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) 400 Creekside Drive Mechanicsburg, PA Water Treatment Plant -The General Contractor did not ensure that the employee working on the top of the grease pit form vibrating concrete were protected from falls, with guard rails, safety nets or personal fall protection, exposing employees to a fall hazard of approximately 9 feet 6 inches high to 6 feet 6 inches high, on or about January 23, 2020. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches: (a) 400 Creekside Drive Mechanicsburg, PA Water Treatment Plant - The employees constructed a form out of plywood and 2x4's . The 2x4's were located vertically at 6 1/2 inches, 42.5 inches and 84.5 inches above the ground. The employees were climbing the 2x4 with a break in elevation of greater that 19 inches, exposing employees to injuries, on or about January 23, 2020. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
