29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: At Sunset Harbor Townhomes, 2800 North Ocean Drive, Hollywood, Florida: On or about 01/29/2020 and at times prior, the employer failed to prevent employees from working near an unprotected wall opening of an elevator shaft, thereby exposing employees to a fall hazard of approximately 20-feet to a lower level.

29 CFR 1926.1052(c)(4): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structural members were not provided between the top rail of the stairrail system and the stairway steps: At Sunset Harbor Townhomes, 2800 North Ocean Drive, Hollywood, Florida: On or about 01/29/2020 and at times prior, the employer did not provide mid-rails on staircases from the ground floor to the third floor level of the structure, thereby exposing employees to a fall hazard of 10 feet to a lower level.
