29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about January 29, 2020, in the warehouse, the employer failed to provide effective guarding to protect employees working at or near the Doringer Model D-350 semi-automatic cold saw. The guarding device for this machine, a two-hand control, did not prevent employees from exposure to the point of operation during the start process, thereby exposing employees to hazards associated with rotating parts.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) On or about January 29, 2020, on the Thread Line, the employer failed to ensure the listed or labeled electrical equipment was used or installed in accordance with the instructions included in the listing or labeling. A Square D Heavy Duty Safety Switch was installed and used on the Landis 16C Dual Head Pipe and Nipple Threading Machine as an on/off switch, thereby exposing employees to hazards including electrocution, electric shock or arc flash.
