Third in Perrysburg, OH: The employer did not ensure that a personal fall arrest system was used by employees performing roofing operations from aerial lifts. Employees were exposed to approximately 25 ft. fall hazards.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. MICAM, LLC dba Imperial Construction Services on or about January 29, 2020, at the worksite located at 143 W. Third in Perrysburg, OH: The employer did not ensure that employees who utilized the JLG aerial lift (model 600A) were trained by a qualified person to recognize and control fall hazards. Three employees worked from the lift without utilizing fall protection.
