29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    On or about January 30, 2020 the employer failed to ensure that machine specific procedures for the Raimann rip saw (model protirip KM 310 M) were developed prior to service and maintenance being performed on the machine. Hazardous energies present on the equipment mechanical, electrical, and pneumatic.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s);    On or about January 23, 2020 the employer failed to ensure that employees tasked with cleaning of the Raimann rip saw (model protirip KM 310 M) operated the energy isolating devices in a manner in which isolated the hazardous energy sources prior to work being performed. Hazardous energy sources present on the rip saw include but are not limited to pneumatic, mechanical, and electrical.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    On or about January 29, 2020 the employer failed to conduct a periodic inspection of the energy control procedures in place at the facility. The company had not performed an annual audit of the program, the procedures, or the personnel tasked with utilizing the control of the hazardous energies.

On or about January 23, 2020, the employer failed to ensure that positive means were in place on the Raimann rip saw (model protirip KM 310 M) to confirm that the hood cover area of the saw was affixed or interlocked so to prevent employees from opening the hood and reaching into the uncovered saw.   adjustments are being made to the machines they control.
