29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On or about January 13, 2020, the employer did not ensure that the point of operation of the TG7-910 Clip-Jig machine, located on the TG7 door line, was adequately guarded to prevent employees from accessing the hazard area at the time of operation. A machine operator suffered an amputation injury after the clip insert forks came down on their right pointer finger.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about January 24, 2020, the employer failed to ensure the work related amputation of an employee that occurred on January 13, 2020 was reported to the Toledo Area Office within 24 hours of receiving knowledge of the event.
