29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about January 28, 2020: a) Two employees performing roofing work on Lot 7, 240 Sage Drive, Canton, Georgia, were exposed to a fall hazard of approximately 21.2-feet and greater while working on an approximately 10/12 slope roof without using fall protection. b) Employees performing roofing work on Lot 47, 243 Sage Drive, Canton, Georgia, were exposed to a fall hazard of approximately 21.3-feet and greater when three employees did not use fall protection and one employee attached his rope at the front of his waist directly to the webbing of a rock climbing harness.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about January 28, 2020: a) Two employees performing roofing work were exposed to a fall hazard of approximately 11-feet when they used a ladder that was not extended at least three feet above the porch roof landing of the house they were working on at Lot 7, 240 Sage Drive, Canton, Georgia. b) Four employees performing roofing work were exposed to a fall hazard of approximately 11-feet when they used a ladder that was not extended at least three feet above the porch roof landing of the house they were working on at Lot 47, 243 Sage Drive, Canton, Georgia.
