29 CFR 1910.178(m)(5)(ii): A powered industrial truck shall not be left unattended when the operator is 25 ft. or more away from the vehicle which remains in view, or whenever the operator leaves the vehicle and it is not in view.  Employees engaged in residential framing activities, working from an elevated work platform attached to the Caterpillar TH360B all terrain forklift, were exposed to struck-by, crushing and fall hazards in that the forklift was left unattended while the operator worked from the elevated work platform more than 25 feet away.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  Employees engaged in residential framing activities using pneumatic nail guns were exposed to struck-by hazards in that eye protection was not provided.

29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:  Employees engaged in residential framing activities were exposed fall hazards and crushing hazards. Employees were exposed in that the scaffold platform was not attached to the forks or forklift.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  Employees engaged in residential framing activities were exposed to falls from approximately 12 feet, in that the aluminum ladder used to access the roof did not extend at least 3 feet from the eave of the roof and was not secured to the building.
