Coast Guard approved ring life buoy with a line attached was not located on each staging alongside of a floating vessel on which work was being performed. In the following locations, ring buoys staged at the docks measured 20-inches: a) Full-service yard dock b) Self-service yard dock Employees perform unloading, launching, and/or repair work on vessels and are at risk of falling into the water.
