The employer routinely fails to assure effective protection against falls as required by the standard, as demonstrated on January 29, 2020, when employees were installing shingles on a residential roof located at 475 College Highway, Southwick, MA with out fall protection. Employees were exposed to fall hazards of more than 15 feet above the next lower level (ground).     Cesar Lopez  was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.503 (b) 1 which was contained in OSHA inspection number 125295, citation number 1, item number 1b and was affirmed as a final order on 9/15/2017, with respect to a workplace located at 17 Leete St., Springfield, MA.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).

Cesar Lopez  was previously cited for a repeat violation of this occupational safety and health standard 29 CFR 1926.1053 (b) 1 which was contained in OSHA inspection number 122643, citation number 1, item number 3 and was affirmed as a final order on 7/17/2017, with respect to a workplace located at 17 Lindale St., Springfield, MA.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).
