29 CFR 1910.136(a): General requirements. The employer shall ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects.  a. On or about January 24, 2020, an employee was exposed to foot crushing, amputation, and struck-by hazards when operating the crane on the HTFL Jumbo section and moving a 2,000 pound steel coil to the down ender without adequate protective footwear with metatarsal guards. The coil dropped on the employees foot causing crushing injuries and toe amputation to the employees foot.

29 CFR 1910.179(b)(8): Designated personnel - Only designated personnel shall be permitted to operate a crane covered by this section.  a. On or about January 24, 2020, a temporary employee was operating a crane on the HTFL Jumbo section without being properly trained or designated as a crane operator. As a result, the employee was exposed to struck-by and crushing hazards and associated crushing injuries and amputation of toe on their right foot.

29 CFR 1904.7(b)(3): How do I record a work-related injury or illness that results in days away from work? When an injury or illness involves one or more days away from work, you must record the injury or illness on the OSHA 300 Log with a check mark in the space for cases involving days away and an entry of the number of calendar days away from work in the number of days column. If the employee is out for an extended period of time, you must enter an estimate of the days that the employee will be away, and update the day count when the actual number of days is known.  Located at 1455 James Parkway, Heath, Ohio 43056:  a. On the 2020 OSHA 300 log, the employer did not ensure the number of days employees were away from work was accurately recorded on the log.  1. On January 24, 2020, a temporary employee was injured while moving a 2,000 pound coil with a crane that resulted in days away from work. The employer did not accurately record the number of days the employee was away from work.

29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  Located at 1455 James Parkway, Heath, Ohio, 43056:   a. On the 2020 OSHA 300 log, the employer did not ensure that all injuries were properly recorded. The two injuries included in the log did not have the nature of the resulting injuries described and completed per Column F instructions.  1. On January 24, 2020, a temporary employee was moving a 2,000 pound coil with a crane and the coil fell off the hook of the crane onto their right foot. The resulting injury was not described in Column F of the log.  2. On February 24, 2020, an employee was injured when their fingers were pulled in-between two rollers. The resulting injury was not described in Column F of the log.
