KC Midstream Solutions, LLC, 101 Queen Creek Rd, Endeavor, PA - On or about January 31, 2020, and at times prior thereto: employees were potentially exposed to fire and/or explosion hazards in that,  written operating procedures for process equipment including, but not limited to, the fired heater, pressure relief valves, the 30,000 gallon hydrocarbon storage tank, and associated piping did not include:   Operating limits;    Consequences of deviation; and     Steps required to correct or avoid deviation.     Safety and health considerations:     Properties of, and hazards presented by, the chemicals used in the process;     Precautions necessary to prevent exposure, including engineering controls, administrative controls, and personal protective equipment;     Control measures to be taken if physical contact or airborne exposure occurs; and    Any special or unique hazards.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
