29 CFR 1926.451(b)(7): On scaffolds where platforms are overlapped to create a long platform, the overlap did not occur only over supports:  (a): Exterior Scaffold 511 Washington Ave- Employees were exposed to fall hazards of approximately 12 feet when working on scaffold platforms that were not properly overlapped. Pumpjack scaffold platforms were not overlapped over supports, on or about January 30, 2020.  There is no abatement certification or documentation required for this item.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.    (a): Exterior North Side- Employees were not provided with proper access to the scaffold. Employees were climbing in and out of the windows of the structure in order to access the scaffold, on or about January 30, 2020.  There is no abatement certification or documentation required for this item.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a): Exterior Scaffold 511 Washington Ave- Employees were exposed to fall hazards ranging from approximately 12 to 25 feet when working on pumpjack scaffolds without proper fall protection. The employees were not utilizing personal fall arrest systems and the pump jack scaffolds were not equipped with proper guardrail systems, on or about January 30, 2020.  There is no abatement certification or documentation required for this item.
