29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:    3910 North Saint Mary's Street: On or about January 21, 2020, and at times prior thereto, employees using Upgear by Werner Roof Anchors that were not secured properly were exposed to a fall hazard, while working on an exhaust system without being trained on the correct use and operation of the anchors in accordance to manufacturers recommendations.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about January 21, 2020, and at times prior thereto, employees were exposed to a fall hazard while using an extension ladder without the side rails extending at least 3 feet above the landing.
