By the elevator shaft on the third floor of a new residential construction located at Lot 6 Vermillion Way, Santa Rosa Beach, Florida, 32459: On or about January 28, 2020, and at times prior, the employer exposed subcontracting employees to an over 25 foot fall potential hazard, in that an employee was installing light fixtures near an unprotected elevator shaft without any caution signs in place, ultimately falling through it.
