The employee was observed working from a makeshift scaffold platform which was being supported by the forks of a JBC 506B Loadall all-terrain forklift. The entire platform was not attached to the forklift. The platform was also moved horizontally while the platform was occupied. The employee was exposed to a fall hazard distance of 11 feet to the ground below.
