29 CFR 1910.179(g)(1)(i): Wiring and equipment did not comply with subpart S of this part. a) Overhead Crane South Bay, on or about January 28, 2020 and at times prior thereto; pendant station wires were exposed due to torn sheathing, exposing operators to electrical hazards and crush-by hazards while hoisting bundles of steel rolls and product in excess of 6 tons or 12,000 pounds. 29 CFR 1910.179(m)(1): Deterioration, resulting in appreciable loss of original strength, was not being carefully observed and a determination made as to whether further use of the rope would constitute a safety hazard: a) On or about January 28, 2020 and at times prior thereto; employer did not ensure that a thorough daily and periodic inspection of overhead shop crane 3093 was completed; deficiencies found on weight bearing wire ropes to include bending, fraying, and waves. b) On or about January 28, 2020 and at times prior thereto; employer did not ensure that a thorough daily and periodic inspection of overhead shop crane 8507 was completed; deficiencies found on weight bearing wire ropes to include waves, rust, flattening and a kink. Note* Abatement certification IS required for this item.

29 CFR 1910.179(n)(1): Crane(s) were loaded beyond the rated load (maximum load for which the crane or individual hoist was designed and built by the manufacturer and shown on the equipment nameplate(s): a) Overhead Crane North Bay, on or about January 28, 2020 and at times prior to, was operated beyond its rated capacity of 7.5 tons, by lifting loads of metal rolls and bundles exceeding 10 tons (or 20,000 pounds). Product loads require a two hoist lift, but because of its position in the warehouse, only one crane is utilized. b) Overhead Crane South Bay, on or about January 28, 2020 and at times prior to; was operated beyond its rated capacity of 6 tons, by lifting metal rolls and bundles exceeding 10 tons (or 20,000 pounds).
