29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:    a) East end of building, north side, on or about 1-31-20:  Two employees were installing final truss bracing while standing in/on the trusses, without fall protection, exposing the employees to a fall hazard of 13 feet.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) South side of building, on or about 1-31-20:  Two employees used a portable fiberglass extension ladder for access to the truss area of a building, with side rails that were 3 inches below the landing surface, exposing the employees to a fall hazard of  13 feet
