29 CFR 1915.152(a): The employer did not provide or ensure each affected employee with the appropriate personal protective equipment (PPE) for the eyes, face, head, extremities, torso, and respiratory system, including protective clothing, protective barriers, personal fall protection equipment, and life saving equipment, meeting the applicable provisions of Subpart I, wherever employees were exposed to work activity hazards that required the use of PPE. Pier: Employees work on and around the pier, such as when launching boats and/or removing them from the water. Personal floatation devices (PFDs) were not required and/or used when working adjacent to the water. Employee(s) are at risk of falling into the water.

29 CFR 1915.158(b)(3): At least one 30-inch U. S. Coast Guard approved ring life buoy with a line attached was not located on each staging alongside of a floating vessel on which work was being performed. Pier: Employees work on and around the pier, such as when launching boats and/or removing them from the water. The pier did not have a ring buoy staged in the area for rescue purposes. Employees are at risk of falling into the water.
