29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:     21 Industrial Rowe, Gardner, MA: On January 31, 2020 and at times prior, employees who operated the Waxing machine were exposed to hazards of uncontrolled energy when they threaded, unlocked the chucks, and wrapped finished rolls on the rewinder.  Employees did not lockout the machine and did not use alternative measures to provide effective protection during machine setup tasks. On January 31, 2020, two employees were exposed with one fatally injured while inspecting the chucks for serial numbers without the machine being locked out or alternative measures being used.
