OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: a) Bay 2, Gallatin Lube Stores, LLC dba Jiffy Lube, Bozeman, MT: On or about January 31, 2020 and at times prior, employees were exposed to struck by hazards while using a Devon Industries, model Blazer 9000 automotive lift to raise vehicles with a defective lift lock. b) Bay 2, Gallatin Lube Stores, LLC dba Jiffy Lube, Bozeman, MT: On or about January 31, 2020 and at times prior, employees were exposed to struck by hazards while using Devon Industries, model Blazer 9000 automotive lift to raise vehicles without annual inspections performed and documented.

The employer failed to ensure the bulk containers were labeled with chemical stored inside for use.
