The employer did not initiate and maintain safety and health programs related to the framing work being conducted by employees.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) YADAICELA GENERAL CONSTRUCTION LLC, located at 868 Peacock drive, Sunbury, OH 43074: On and preceding February 3, 2020,  the employer did not instruct employees in the recognition and avoidance of unsafe conditions and the procedures to be followed in order to minimize these hazards. Framers were installing walls and wood decking on the second floor of a home that varied from 10-12 feet with the employees working at different levels. The employees were not utilizing proper eye or fall protection which exposed them to falls up to 12 feet and struck by hazards to the eyes.

Framers were installing walls and other framing pieces on the first and second floor of a two story residential home. The employees on the ground level were not utilizing head protection which exposed them to struck by hazards to the head.

Framers were installing joists and wood decking using a pneumatic nail gun and a circular saw to cut the pieces. The employees were not utilizing eye protection which exposed them to struck by hazards to the eyes.

Framers were utilizing an extension ladder to access the second floor of a two story residential home which was positioned approximately 3 inches below the upper landing they were accessing.  The following condition exposed employees to falls up to 12 feet.

Framers at the site were using an 8 foot self-supporting ladder as a single ladder, in the closed position leaning against a wall.  The following condition exposed employees to falls up to 8 feet.
