29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Evans Roadhouse Restaurant & Pub, Drums PA: Employees were using a 20ft. Werner 225lb. portable extension ladder to gain access to the 5:12 pitch roof to perform roofing activities. The portable ladder used for access to the upper landing surface and the ladder's length, the ladder side rails did not extend at least 3 feet (.9 m) above the landing surfacing being accessed on or about February 3, 2020.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) Evans Roadhouse Restaurant & Pub, Drums PA: Three employees on a steep roof (5:12 pitch) approximately 13 feet 5 inches with unprotected sides and edges 6 feet or more above a lower level were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems on or about February 3, 2020. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED B.J. Roofing & Siding, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1418332, citation number 1, item number 1 and was affirmed as a final order on September 10 2019, with respect to a workplace located at 202 N. Main St, Plains, PA .
