29 CFR 1910.28(b)(1)(i):  Each employee on a walking/working surface having an unprotected side or edge which was four feet (1.2m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system:    On or about 1/28/2020, employees of Middletons RV, Inc. located at 3441 US Highway 67, Festus, Missouri were exposed to fall hazards.  Employees would access the top of recreational vehicles and campers to perform work at heights ranging from 12-14 feet above the lower level without the use of fall protection.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:      On or about 1/28/2020; employees of Middletons RV, Inc. located at 3441 US Highway 67, Festus, Missouri were exposed to stuck-by hazards.  In the maintenance area the work rest on a Craftsman 8 inch variable speed grinder, model no. 152.211620, serial no. 0722-W was adjusted to greater than 1/8 inches to the abrasive wheel.

29 CFR 1910.215(b)(9):  The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    On or about 1/28/2020, employees of Middletons RV, Inc. located at 3441 US Highway 67, Festus, Missouri were exposed to struck-by hazards.  In the maintenance area the tongue guard installed on a Craftsman 8 inch variable speed grinder, model no. 152.211620, serial no. 0722-W was inadequately adjusted. The distance between the abrasive wheel and the metal housing was greater than 1/4".

The employer did not ensure that a written hazard communication program was developed, implemented and maintained.  Employees were exposed to Toluene, Acetone, Cumene, Naphthalene, and other hazardous chemicals.

The employees had not been provided effective information and training on the hazardous chemicals used in their work area upon initial assignment, exposing employees to chemical hazards.
