29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):      (a) On or about January 28, 2020, the employer did not ensure that its employees were protected from falls while working at heights greater than 6 feet. An employee of M K Construction, LLC was exposed to fall hazards greater than 9.5 feet while engaged in residential siding activities with no fall protection systems in place.       Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").     .

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:    (a) On or about January 28, 2020, the employer did not ensure that ladders were used for the purposes for which they were designed. An employee of M K Construction, LLC was exposed to fall hazards greater than 9.5 feet. Employee used an  A-Frame ladder folded as a single ladder leaning against a wall  in order to reach areas of a residential roof while conducting siding activities. A-Frame ladders are not designed to be used folded as single ladders. 29 CFR 1926.1053(b)(7): Ladder(s) that were unsecured or not provided with slip-resistant feet were used on slippery surfaces:  (a) On or about January 28, 2020, the employer did not ensure that ladders used on slippery surfaces were secured or provided with slip-resistant feet. An employee of M K Construction, LLC was exposed to fall hazards greater than 9.5 feet. Employee used an extension ladder placed on a slippery surface with the slip-resistant feet folded back. Ladder was used to access a residential roof to conduct siding activities.
