An employee of M K Construction, LLC was exposed to fall hazards greater than 9.5 feet while engaged in residential siding activities with no fall protection systems in place. An employee of M K Construction, LLC was exposed to fall hazards greater than 9.5 feet. Employee used an A-Frame ladder folded as a single ladder leaning against a wall in order to reach areas of a residential roof while conducting siding activities. A-Frame ladders are not designed to be used folded as single ladders. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

An employee of M K Construction, LLC was exposed to fall hazards greater than 9.5 feet. Employee used an extension ladder placed on a slippery surface with the slip-resistant feet folded back. Ladder was used to access a residential roof to conduct siding activities.
