29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) On or about 01/29/20, on a roofing job site, at a house at 422 Colvin Ave., Buffalo, NY: One employee was exposed to a potential fall of approx. 18 ft. from the aluminum pik of a ladder jack scaffold erected on the north side of the house. The employee was doing roofing work on the lower area of the roof while standing on the pik of the scaffold, and no fall protection system was utilized. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 01/29/20, on a roofing job site, at a house at 422 Colvin Ave., Buffalo, NY: Two employees engaged in roofing work were exposed to a potential fall of approx. 20 ft. from the steep roof on the north side of the house. The employees were not utilizing fall protection. Pitch of the roof was 8 in 12. NO ABATEMENT CERTIFICATION REQUIRED
