29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling:    (a) Facility, 42 84 Drive, Eighty Four, PA:  On or about February 3, 2020 a section of the platform at the grinder was missing exposing employees to a fall hazard of approximately 5 1/2 feet to the floor.  Employees work on the platform to feed plastic to the grinder and are exposed to fall hazards.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) Facility, 42 84 Drive, Eighty Four, PA:  On or about February 3, 2020 the employer had not established an energy control program consisting of energy control procedures, employee training, periodic inspections or devices to control energy sources.  Employees service and/or clean the grinder and there are no procedures in place to control the energy nor have employees been trained to ensure they are aware of what energy sources power the equipment and the methods to control such sources.

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck  (a) Facility, 42 84 Drive, Eighty Four, PA:  On or about February 3, 2020 employees who operate the Toyota and CAT powered industrial trucks on a daily basis were not provided training, nor was an evaluation performed, prior to permitting employees to operate the equipment.
