29 CFR 1910.134(e)(6)(i): The employer did not obtain from the physician or other licensed health care professional (PLHCP) a written recommendation regarding the employee's ability to use the respirator: (a) On or about 2/3/2020 - site, a written recommendation had not been obtained for employees wearing respiratory protection such as 3M R95 and Honeywell Full-Face and exposed to 2,4 Toluene-Diisocyanate (TDI) during the performance of maintenance as well as emergency response activities.

29 CFR 1910.1200(f)(6)(i): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(i) through 29 CFR 1910.1200(f)(1)(v): (a) On or about 2/3/2020 - F line, the four (4) pods containing color and activators were not labeled to identify the hazard and content. (b) On or about 2/3/2020 - Mezzanine above F line, the enclosure was not labeled and signs were not posted to alert employees of the presence of chemicals in tanks. The Day tanks containing 2,4-Toluene-Diisocyanate (TDI) and polyol were positioned behind the enclosure.
