29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a) On or about January 31, 2020, and times prior to, authorized employees were exposed to a caught in hazard, amputation hazard by not being trained on the hazard's of energy control procedures when setting up and doing adjustments to the Vits 61, Vits 66 and Vits 3 in the production area. b) On or about January 31, 2020, and times prior to, authorized employees were exposed to a caught in hazard, amputation hazard by not being trained on the hazard's of energy control procedures when setting up, webbing or doing adjustments to the Laminating machine in the production area.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets. On or about January 28, 2020, and times prior to, the employer failed to fully enclose the chains on sprockets on Vits 66 machine that was below seven feet.
