29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  (a) Center of building - On or about February 4, 2020 The employer allowed an employee to work from the wooden scaffold walkway that was 5 5/8 inch wide exposing, employee to fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Front of house - On or about February 4, 2020 the employer did not ensure employees working from the top of the framed walls above 6 feet height were provided a means of fall protection, exposing them to fall hazards.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    (a) North side of house and inside of house - On or about February 4, 2020 the employer did not ensure employees using ladders to gain access to the top of the wall extend 3 feet above the access point and were not secured to prevent movement, exposing employees to fall hazards.
