From working surfaces of with heights approximately 13.6 feet.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   On or about February 4, 2020 at the job site located at 6188 Swan Lake Cir NW, Canton, Ohio 44718; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:   On or about February 4, 2020 at the job site located at 6188 Swan Lake Cir NW, Canton, Ohio 44718;  during the installation of roofing material on a residential roof, the employer did not ensure that employees utilized stepladders (self-supporting ladder) as they were designed for use. Employees utilized stepladders as a non-self-supporting ladder on a roof to gain access to an elevated working surface.
