1926.21(b)(2) The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: 29 CFR 1926.652(e)(1)(i): Members of support systems were not securely connected together to prevent sliding, falling, kickouts, or other predictable failure: a) On February 3, 2020, the employer failed to ensure a top horizontal supporting member was missing the pin and keeper to secure the connection to the side plate of the protective system, thereby exposing employees to cave-in hazards while working in a trench approximately 7 feet in depth.
