29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe: a) New residential construction located on Lot #2, First and Second levels: On or about 02/05/2020, and at times prior, the employer exposed employees to severe laceration/amputation hazards while operating circular saws, in the all three saws used on site had inoperable lower guards as the lower guard was shimmed to the upper guard to prevent the lower guard from retracting after a cut was made.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) New residential construction located on Lot #2, First and Second levels: On or about 02/05/2020, and at times prior, the employer exposed employees to fall hazards of approximately 21-feet, 5-inches to the lower level, in that fall protection was not used while walking the outside top plates installing roof trusses.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: a) New residential construction located on Lot #2, First level: On or about 02/05/2020, and at times prior, the employer failed to inspect all of the electrical cords prior to the start of the work shift, in that a 50-ft. extension cord used to power a Makita Circular had a damaged protective casing and a damaged strain relief which was not repaired or removed from service.
